How to get there and start the quest:

head north from Sanctuary (or just look at your Pip-Boy and see the mark on the map);
go north from the mark on the map;
make sure you have enough water (5 or more);
look at the skeleton in front of you and examine things.

Settlements range from small to medium in size. Most of them are located in such a way that they are near each other in the city, but for the most part very far from the Commonwealth.

However, some of them are also scattered throughout the Wasteland, including fire towers.
