The Enhancement System.
Redefines all Relic Blades from scratch to be useful / gives you access to NPC weapons only / allows you to upgrade all Relic Blades, Kaer Morhen DLC and Armor, DLC and Witcher Crossbows, Dream Cobra and Vesemir / adds dozens of new loot to the world / changes some vendors / adds a lot of quest rewards / increases the Gwent bet to 50 / adds new animations / effects, etc. For a more comfortable game, use the full translation into Russian.
