How Many Endings Are There in A Plague Tale: Requiem
No matter how much the game tries to motivate you to avoid random deaths, the number of corpses will not impact the ending.
If you've been paying attention, you'll realize towards the end which direction the story is heading. With each chapter, the situation worsens, and only one person is to blame — Hugo. Despite Amicia and other characters trying to convince the boy that the rat tsunami isn't his fault, it's actually his uncontrollable nature that's the reason why ordinary people suffer and die by the hundreds.
In Chapter 16, the main character enters a sort of otherworldly reality, resembling Hugo's subconscious. The boy tries to convince his sister that she has been making the same mistake all along — refusing to give up. A deep philosophical meaning is also reflected in the gameplay. Initially, you need to move in the opposite direction of the birds pointing the way, and then allow the rats to kill the girl. An excellent plot twist, making you go against all the knowledge and skills acquired throughout the game.
Ultimately, Amicia is faced with a choice — to kill her brother or not. Accordingly, there are two endings, directly depending on your last decision.
How to Unlock the Secret Ending
It seems that killing Hugo with a sling is the only available option. Amicia, along with Lucas, stands opposite the boy while swarms of rats continue to destroy all living things. A Plague Tale: Requiem is a linear game, so you wouldn't expect alternatives to such a finale. However, one does exist.
In the case of Amicia personally killing him, she realizes that ending the entire nightmare is only possible by ceasing to fight for her brother. This is the sacrifice that must be made for the greater good, only then will the rats calm down and stop harming people. You need to load the sling with a regular stone and shoot the boy in the head. Almost immediately, the final 17th chapter begins, where an older Amicia meets with Sophia. They discuss future plans, and we understand that the heroine is forever traumatized by what happened, but wants to help the next carriers of the disease.
If, however, in Chapter 16 you do nothing for 2 minutes and don't shoot from the sling, a cutscene will trigger where Lucas shoots instead of the girl. The boy will take a crossbow and do what Amicia couldn't bring herself to do.
Later, when the heroine discusses the young alchemist with Sophia, she will say that Lucas won't come because Amicia still can't forgive him. The boy must be traveling and actively learning. In the main ending, Lucas will also be acquiring new knowledge, but there will be no conflict with Amicia.
