How to Play Quarantine Zone

A zombie apocalypse has broken out in the city, and the military has set up an evacuation camp. People come here seeking shelter, food, and medicine. Some are healthy, some are sick, and some are infected. Your job is to spot the virus and decide where to send each patient: the living area, quarantine, or the elimination block. The main goal is to gather as many survivors as possible for evacuation.

Most of the time, you’ll be identifying symptoms from three categories: illness, illness or infection, and infection. Based on the combination of virus signs, you’ll decide whether to let someone into the shelter, place them in quarantine to watch for symptoms, or send them for elimination if there are clear signs of becoming a zombie.

The most important rule is to never let an infected person into the living block. At night, an infected person will turn into a zombie, killing and infecting several others. You also need to keep an eye on your base’s resources: food, medicine, and fuel. Constant upgrades are essential to avoid overcrowding, hunger, or running out of supplies.

Day 1

The game starts in your tent after the opening cutscene. Here you’ll find a bed where you can end your shift, which is how you progress from one story chapter to the next. First, pick up the radio from the table — this is how you’ll receive instructions from the Major.

Leave the tent and head to the checkpoint to meet Georgie. Listen to the briefing and call in your first patient, then pick up the flashlight. Always keep it on during examinations, since many visual symptoms aren’t visible in natural light. You can also use it with other tools, even when your hands are full.

Symptom Categories

During examinations, you need to identify signs of illness and decide where to assign the patient.

There are three types of symptoms:

  • Illness. Green: Safe symptoms that might look like signs of disease, but aren’t actually dangerous. You can send these people to the living area.
  • Illness or Infection. Brown: Early stage infection or symptoms of other illnesses. These signs are similar to infection. In this case, place the patient in quarantine. On the next day or later, the symptom will either disappear or progress to the “Infection” stage.
  • Infection. Red: Clear signs of the zombie virus. Infected people must be sent to the elimination block. If you send them to quarantine or the living area, the disease will spread to other survivors.

At the start of the game, your handbook lists symptoms you can identify by appearance:

  • Dirt: This isn’t a symptom and won’t show up in the handbook. The problem is that stains, smudges, and dust can hide signs of infection. Some types of dirt even look like scratches, bruises, or necrosis. So take your time and learn to tell the difference between real symptoms and just dirt.
  • Dried blood: Counts as dirt. Blood stains on skin or clothes can easily be mistaken for scratches.
  • Tattoos, scars, birthmarks: Symptoms can be hidden on these skin markings. For example, some tattoos can cover up bites or necrosis.
  • Vitiligo: Some people have multiple pigment spots, making their skin look a bit like cowhide. This is not a symptom and doesn’t need to be investigated.
  • Yellow eyes. Green: Yellowish sclera around the iris.
  • Conjunctivitis. Brown: Pinkish sclera with redness along the eyelid edges.
  • Red eyes. Red: The whole sclera is bright red due to burst blood vessels.
  • Freckles. Green: Multiple pigment spots of different sizes on the cheeks and nose bridge. They don’t extend into the nasolabial triangle.
  • Bruises. Green: Hematomas of various colors, from dull green to purple. Can appear together with scratches and be all over the body. Some bruises look like necrosis, so always check the handbook images.
  • Scratches. Brown: Multiple cuts that could let infection in. Often found together with bruises and can be all over the body.
  • Bite. Red: Mark from upper and lower jaw. The wound edges have characteristic bruising and mild inflammation. Can be found anywhere on the body.

During examinations, always mark every symptom you find. If you send someone to the correct place, you’ll get a “Flawless Inspection” bonus. Otherwise, you’ll see “Poor Inspection.”

On the first day, you’ll see five people: two healthy, one for quarantine, and the rest for elimination. Once you’ve sent the last patient where they belong, return to your tent and go to bed to start the next shift.

Day 2

At the start of each shift, a “Finances” window appears. Here you can see your base’s income and expenses. Your earnings depend on how you assign patients and record their symptoms. Each day, $500 is spent on maintaining the checkpoint. The next table, “Deaths in the Block,” shows how many people died because of mistakes in the previous shift:

  • Infected allowed into the living area
  • People harmed by the infected
  • Deaths from an overcrowded shelter
  • Deaths from starvation
  • Healthy people sent to the elimination block

If you sent people to quarantine in the previous shift, you’ll need to check the isolation ward before admitting new refugees. Overnight, symptoms may disappear or get worse. If they’re gone, send the person to the living area; if they’ve gotten worse, send them for elimination. If someone turns into a zombie, everyone in that cell will get infected, so you’ll have to decontaminate the room by pulling the lever to the right of the door. Sometimes a person might stay in isolation for several days.

How to Use the Thermopulse Meter

Go to your partner and pick up the thermopulse meter. This tool lets you check a patient’s pulse and temperature. If there are abnormalities, the person may hiccup or cough. A new section will appear in your handbook:

  • High temperature (37.0°-41.0°). Brown: Normal temperature is around 36°C. An elevated temperature means there’s inflammation in the body, but it’s not a marker of infection.
  • Very high temperature (41.0°-45.0°). Red: With hyperpyretic fever, the person is close to death because their proteins are breaking down. In this case, it’s a sign of infection.
  • Elevated pulse (120-180). Brown: Persistent high tachycardia means severe stress or an ongoing infection.
  • Very high pulse (180-200). Red: At this pulse, people usually have a heart attack or stroke. Here, it’s a clear sign of the virus.

Sometimes the pulse or temperature will be right on the edge of normal, which can make things tricky. In this case, hold the thermopulse meter for about ten seconds and watch the readings. Focus on the number before the decimal: if it stays the same while the digits after the decimal fluctuate, make your decision based on that. For example, if the temperature stays between 41.06-41.30, it’s the red “Very high temperature” symptom.

Note: If a person has two brown symptoms, send them to quarantine. If the symptoms are still there the next day, eliminate the infected person.

Most of the time you’ll see random people with random symptoms. Occasionally, characters from famous games will show up — for example, a bearded man with a pot on his head who mumbles strangely is Crazy Dave from Plants vs. Zombies.

How to Defend the Base

At the end of your shift, return to bed. After the night, two waves of zombies will attack the checkpoint. Go to the laptop and take control of the drone. The drone is equipped with a machine gun, a grenade launcher, and a heavy cannon. There are lots of fuel barrels in front of the gate.

Base defense is a mini-game where you have to shoot down waves of infected. Use the machine gun against regular zombies, and take out mini-bosses with heavy weapons or by blowing up barrels. If you succeed and the walls aren't damaged, you’ll earn a cash reward.

Day 3

The morning starts with base management. You need to make sure you always have enough food, supplies, medkits, and power. Hunger leads to death, the sick will die or turn into zombies, and healthy people will become sick. Any building without power will stop working. Try to earn as much money as possible by following all procedures correctly during inspections. Upgrade your facilities as soon as you can: generator, tents, quarantine, checkpoint, and so on.

Head to the checkpoint, check the quarantine area, and start your examinations. Today you’ll have five patients. At the end of the day, go back to bed. You can use quarantine as temporary housing if the living area is full. But remember, if there’s an infected person in quarantine, healthy people there will become sick.

Day 4

There won’t be any incidents at night, but in the morning the power will go out. Open the base menu and buy as much fuel for the generator as you can. Leave your tent, go to the living area, and flip the switch. Now open your tablet and go to the “Upgrades” section to level up your camp. Then, buy upgrades for the generator, and you can also improve other base facilities.

How to Earn Research Points

When you thoroughly examine patients, you earn research points. These are used to upgrade your zone, which allows you to improve base facilities. You can also use these points to upgrade medical equipment. Mark every symptom, even if it’s obvious the person is infected, or you won’t get the bonus for a quality inspection.

How the Reflex Hammer Works

Restock your food and medical supplies. Return to the checkpoint, enter quarantine, and talk to Georgie. You’ll now get the reflex hammer tool and some new symptoms:

  • Zero reflex. Green: Normally, elbows and knees move slightly backward. Sometimes, instead, they just twitch a little due to a dulled reflex or don’t move at all.
  • Hyperaggression. Brown: Hitting any limb causes the person to attack the player. Don’t fight back—just send the person to isolation.
  • Crossed reflex. Red: Due to severe motor dysfunction, hitting the right limb causes the left to jerk, and vice versa.
  • Slack jaw. Brown: The mouth is always open and droops to one side. This can be seen in people with chronic neurological illnesses, but it can also signal infection.

If you see both a slack jaw and a crossed reflex, send the infected person to the elimination block. If the arm doesn’t move back when struck, mark zero reflex—even if the jaw is slack. You’ll have five patients this shift.

Day 5

Today you’ll have about ten patients. At the end of the shift, return to your tent to begin evacuation. Two trucks will arrive at the base—one with soldiers, one with scientists. You’ll need to assign survivors to each. If you meet certain quotas, you’ll earn research points from the medics or money from the soldiers. The more people you evacuate, the more rewards you get. To achieve the good ending, you must evacuate at least five people.

Day 6

The daily report will now include an “Evacuation” window, showing casualties and who was sent or left behind. Buy fuel for the generator, then visit quarantine. Get the scanner from your colleague—this tool lets you see through clothing. You’ll also see new symptoms in the “Appearance” category:

  • Red skin. Green: Reddish circles on the skin, usually caused by allergies or chronic local blood vessel dilation.
  • Pale skin. Brown: The whole body has an unhealthy color—ranging from pale to gray or even purple. This is a sign of serious health issues or possible infection.
  • Bruise. Brown: Bright red or maroon bruises in specific spots. Can appear anywhere on the body. Most often it’s just an injury, but sometimes it’s a sign of infection.
  • Green skin. Red: The body takes on a corpse-like color, from yellowish to green. This is a clear sign of infection.
  • Necrosis. Red: Large black bruises on different parts of the body, often with yellow or green pus-filled sores.

Day 7

On the last day of the first week, a bulletin board will appear at the checkpoint. Here you’ll find extra tasks—completing them earns you rewards. There are a lot of assignments in the game, so we’ve prepared a separate guide for them.

How Contraband Works

First, check your base supplies and the infirmary, then get to work. Now you’ll need to inspect refugees’ bags:

  • Personal items: Cameras, books, teapots, and so on—these are allowed without any issues.
  • Food, water, fuel, and medicine: Also allowed, but you can use these supplies to restock your base resources.
  • Weapons and ammo: Knives, grenades, rifles, pistols, grenade launchers, and any other weapons are strictly forbidden. Confiscating them will earn you a cash bonus.
  • Human remains: Skulls, bones, and other remains are considered a biological hazard. In this case, the patient is considered infected and must be eliminated.
  • Gold figurines: You can take these statuettes. Each one is worth $500. There are 14 in the game, and four of them can randomly be found in refugees’ bags.

You can only open a refugee’s bag during an inspection. Once they’re sent to the living area, quarantine, or for elimination, you can’t take their items. If a patient has luggage, mark it on your tablet. If you find contraband, select weapons or infected items; otherwise, choose luggage.

Day 8

At night, zombies will attack the base. The first wave will be regular mobs, and the second will have large mutants and a few giants. In the morning, the major will ask you to find the vandal who painted six tags on the walls:

  1. Opposite the hero’s tent.
  2. In the garage, near the porta-potty.
  3. Enter the lab area, turn right, and go to the transformer booth.
  4. Back side of the hero’s container.
  5. Watchtower in the elimination zone.
  6. Under the sign pointing to the lab.

The tags appear one after another. After the sixth, go to the living area and talk to Abe Crow, then find the scientist in the blue suit in the garage warehouse. Look for the troublemaker among the survivors—he’ll have a big red spot that’s visible under a flashlight. You can take money or kick the vandal out—the choice doesn’t affect anything.

How the Lab Works

Return to the checkpoint and start your inspection. The first patient will have symptoms not found in your database, so you’ll need to take them to the lab for research:

  1. Follow the patient to the lab and use the laptop.
  2. Use the laser to cut open tissue and remove the organ highlighted in green (don’t touch the red ones). Try not to damage anything else. If you’re quick and careful, you’ll get a chestplate as a reward.
  3. Place the organ in the grinder.
  4. Take a test tube.
  5. Put it in the analyzer on the table to the right.

This is how you’ll discover new symptoms and earn research points. If you send someone for research, make sure to mark “Unknown symptom” on your tablet before heading to the lab.

Rash. Brown

Pancreas. Characterized by multiple pimples. It can be localized to certain areas or spread across the whole body. Usually, it’s an allergic reaction or a dermatological condition. This symptom can progress to necrosis.

Some symptoms will need to be identified in the lab. These are usually very noticeable and visible from a distance.

Blood from the Mouth. Brown

Tongue. A visible bloodstain under the lower lip. This could be caused by an injury or indicate a virus that destroys the gums of the infected.

On this day, a man with bruises and a new symptom will show up for one of the tasks.

Aggression. Brown

Left kidney. The face always has a distinct angry look—corners of the mouth turned sharply down, teeth clenched. The patient is constantly swearing and may attack on the way to quarantine or the elimination block. Either the person has experienced severe stress, or the virus is triggering an adrenaline rush from the adrenal glands.
