This retro-style action platformer with RPG elements follows a father who journeys into hell to save the souls of his wife and child. Insanity's Blade tells the story of the creation of a sword possessed by Thurstan's grief and hatred, and its journey through the underworld.

Players fight enemies, traverse stages, take on side quests, climb walls and jump, while collecting weapons and upgrading abilities. Story mode uses cutscenes between levels to tell the tale, while Arcade mode focuses on continuous combat. Local co-op is also available, with the second player taking control of different supporting characters.
