This 3D action-adventure with Metroidvania-like elements follows Twisty, a floating brain with teeth trying to escape from an insane asylum. He battles those intent on keeping him imprisoned, while the grim setting is paired with dark humor and parodies of horror tropes and games of the past.
Players explore the asylum, fight enemies in third person, use stealth, collect items, and acquire power-ups. Twisty can also gain minions, and the gameplay includes nods to classic action games.
