The game returns in a refreshed format, preserving the iconic atmosphere of the dark shooter while adding a modern twist. Purgatory becomes the arena for merciless battles, where hordes of demons and colossal monsters bar your path. Play solo or in co-op with up to three players, exploring gothic regions and using both the new and classic Painkiller arsenal. Jumping, dodging, and the ability to latch onto objects make every battle dynamic and spectacular.

The story revolves around the fallen angel Azazel, who has decided to unleash armies of demons upon Earth. As one of the champions, you must navigate the armies of darkness, battle unique enemies, and encounter three Nephilim—Azazel's monstrous creations. The fate of humanity and redemption intertwine in a brutal struggle where every step becomes a test.
