Karlach Features and Soul Coins
Karlach is a Zariel Tiefling, so she has several useful racial features, including resistance to fire damage. This fits her Barbarian playstyle well, but the most important unique mechanic for Karlach is tied to Soul Coins.
Soul Coins can be found throughout the Sword Coast and used to empower Karlach in combat. When Karlach uses a Soul Coin, she gains the Infernal Fury effect, allowing her to deal extra fire damage while she is Raging or when her hit points are low.
Because this Karlach build relies on aggressive melee combat, Soul Coins are best saved for difficult fights, bosses, and encounters where you need her to deal as much damage as possible.
How to Play Karlach as a Barbarian
Karlach works best as an aggressive front-line Barbarian who stays close to enemies, absorbs damage, and deals heavy melee hits. In most fights, her main job is simple: enter Rage, rush into melee range, and focus the most dangerous enemy before they can threaten the rest of the party.
There are two main ways to play Karlach: as a durable tank or as a high-damage melee fighter. This build focuses more on damage, but Karlach is naturally tough enough to survive on the front line if you position her correctly.
Rage is the core Barbarian mechanic. It increases Karlach’s melee damage and helps her withstand physical attacks, but Rage charges are limited, especially early in the game. Do not waste Rage on small encounters. Save it for boss fights, large enemy groups, and battles where Karlach needs to survive several rounds under pressure.

Important Note: Karlach should not wear heavy armor. If a Barbarian wears heavy armor, the character becomes Rage Impeded: Rage no longer grants bonus damage, resistance to physical damage, or Advantage on Strength checks and saving throws. Since Rage is the core of any Karlach Barbarian build, heavy armor is not worth it.
For armor, use medium armor. It gives Karlach solid protection without disabling Rage. If you want to make her more defensive, you can also equip a shield, especially when you need her to survive on the front line instead of dealing maximum damage.
For weapons, pay attention to the weapon damage dice. Karlach benefits greatly from strong two-handed weapons, especially later in the game when she gains features that improve critical hits.
Between a weapon that deals 2d6 damage and a weapon that deals 1d12 or 1d10 damage, this build prefers the larger single weapon die. This works better with Barbarian critical-hit features, because an additional weapon die is added when Karlach lands a critical hit.
In practice, this means Karlach should usually prioritize powerful melee weapons with a large single damage die, such as greataxes. Greatswords and mauls are still strong options if they have better bonuses, but for critical-hit scaling, a weapon with 1d12 or 1d10 damage fits this build better.
Best Tank Subclass for Karlach at Level 3
If you want to play Karlach as a tank, choose the Wildheart subclass at level 3. This is the best Barbarian subclass for survivability because it improves Rage and gives Karlach stronger defensive options in the middle of combat.
For this version of the Karlach Barbarian build, choose Bear Heart. While using Rage: Bear Heart, Karlach becomes much harder to kill and can withstand heavy damage while staying on the front line. This is exactly what you want from a tank: she draws attention, survives dangerous hits, and gives the rest of the party more freedom to attack safely.
Bear Heart also gives Karlach Unrelenting Ferocity, a healing action that helps her recover HP during combat. It is especially useful in long fights, boss encounters, or situations where your healer cannot reach her in time.

Best Tank Feat for Karlach at Level 4

This feat works especially well with Bear Heart. Karlach already gains strong survivability from Rage: Bear Heart, and Tough gives her an even larger health pool to survive long fights, boss encounters, and situations where several enemies focus her at once.
If Karlach is the main tank in your party, extra HP is one of the safest upgrades you can take. It gives the rest of the group more time to deal damage, heal, reposition, or control enemies while Karlach absorbs pressure.
Best Animal Aspect for Tank Karlach at Level 6
At level 6, choose Aspect of the Beast: Stallion for a tank Karlach build. This Animal Aspect gives Karlach temporary hit points whenever she uses Dash, making her harder to kill while she moves into melee range or repositions during combat.
This works well with a front-line Barbarian playstyle. Karlach often needs to close the distance, rush toward dangerous enemies, or reposition during combat. With Aspect of the Beast: Stallion, every Dash becomes both a movement tool and a defensive bonus.

At level 10, choose Aspect of the Beast: Chimpanzee for the tank Karlach build. This Animal Aspect gives Karlach resistance to fall damage, which becomes very useful in late-game fights where enemies can push, throw, or knock characters from high ground.
Aspect of the Beast: Chimpanzee also gives Karlach a funny but useful combat trick: camp supplies thrown by her can Blind the target. This gives you another way to weaken dangerous enemies, especially when Karlach needs to control the battlefield instead of only absorbing damage.
For a tank Barbarian, this is a strong utility choice. Karlach already has enough survivability from Bear Heart, Tough, higher Constitution, and Aspect of the Beast: Stallion, so Chimpanzee adds extra safety and a small control option for difficult encounters.
Best Tank Feat for Karlach at Level 12
At level 12, choose Ability Score Improvement again and increase Karlach’s Constitution to 20. This is the final step for the tank version of the Karlach Barbarian build, giving her the largest possible health pool from ability scores.

While Frenzied, Karlach gains access to powerful bonus-action attacks such as Frenzied Strike and Enraged Throw. This gives her more ways to deal damage every turn, finish weakened enemies, or throw enemies and objects across the battlefield.
The idea is simple: more attacks mean more damage. A Berserker Karlach build works best when you want her to rush into melee range, stay aggressive, and remove dangerous targets as quickly as possible.
Best Damage Feat for Karlach at Level 4
At level 4, choose Great Weapon Master for the damage version of this Karlach Barbarian build. This feat is one of the best ways to increase melee damage, especially when Karlach uses heavy two-handed weapons.
Great Weapon Master gives Karlach two important bonuses. First, when she lands a critical hit or kills a creature with a melee weapon attack, she can make another melee attack as a bonus action. Second, she can take a -5 penalty to attack rolls to gain +10 bonus damage.
