The sprawling story of Pathologic 3 forces players to make tough moral choices that affect the final outcome of the game and the fate of the town-on-the-Gorkhon. In this guide, we’ll break down the requirements for unlocking all the main endings in the game.

“Escape” Ending

To escape from the town-on-the-Gorkhon, you’ll need to complete a series of steps:

  1. On the fourth day, save Gryph from the bandits at the Warehouses.
  2. Next, complete the series of quests with the switchman.
  3. Go to the cemetery, talk to the caretaker Laska, and look for their son’s grave.
  4. Talk to Laska again, then return to the switchman. Convince him that his son is alive.
  5. Don’t forget to speak with Gryph again on the same day at Isidor’s house.
  6. Then, complete all tasks related to Gora: find him and bring a bottle of tvirin on the seventh and eighth days. Ask the steppe dweller about the journey.
  7. On the ninth day, save Katerina Saburova and ask the commandant for a small favor, referencing the help you gave his wife. This way, the patrolmen and your other wards won’t interfere with Dankovsky’s escape.
  8. Finally, on the tenth day, you’ll receive a message from Gryph and find him once again at the same house. Agree to escape from the plague-ridden town and, at the exit, talk with Maria Kaina. If you choose only positive dialogue options, you’ll unlock the “Heartburn” achievement.
  9. For the ending, respond coldly to all questions and run to the train in the steppe.

Most other endings depend on your main story choices.

“Rot” Ending

  • Cancel Burakh’s or Aglaia Lilich’s storyline using the Rat Seer.
