Victims are common in Pathologic 3 is the Plague spares no one, and your goal is to stop it. However, most deaths can be prevented. You’ll need to be in the right place at the right time, set your priorities carefully, and sometimes sacrifice other activities. In this guide, we’ll break down how to save key story characters.

Day 3 — Saving Stanislav Rubin

On the third day of the game, you get your first chance to save one of the important characters—Stanislav Rubin.

After you wake up, you’ll find yourself in the Abyss. Go down the stairs and head outside. As soon as you exit, make your way toward the warehouse district. The game will offer you alternative activities at this point, but if you want to save Rubin, you should ignore them.

You’ll have to go through an infected district. Along the way, you’ll meet Clara, talking to her is optional, but it’s recommended not to skip this conversation.

Once you reach the warehouses, you’ll enter a dangerous area patrolled by guards. They’ll stop you and warn you about an approaching group of bandits. During the conversation, you’ll receive your first firearm—a revolver and some bullets.

As you move deeper into the area, follow the pipes and narrow passages between buildings. Stick to the right side until the path leads you to the boiler house. Two bandits will stop you at the entrance. The dialogue will always lead to a fight, and the best way to finish it is to shoot one of the attackers. The other will retreat, ending the conflict.

Go to the boiler house door and talk to Rubin. This conversation completes the story quest and counts as your first character rescue. Rubin will play an important role later, so make sure to save him before moving on.

Day 4 — Saving Stanislav Rubin a Second Time

On the fourth day, Rubin’s situation becomes critical. If you saved Rubin earlier, head straight to the city cemetery after waking up.

At the entrance, you’ll meet a boy who tells you about a strange ringing near one of the graves. He hints that you should look for a grave with a bell.

Before you start digging, you’ll need to find a shovel. It can be in one of two places: next to the stone pillars to the right of the entrance, or near the nearby stone building.

After picking up the shovel, return to the main path and head deeper into the cemetery, keeping to the left side. In the far section, you’ll hear a bell ringing and spot a fresh grave. Start digging, then talk to Rubin.

It’s important to finish this episode before 3 PM in-game time. If you’re late, the character will die and you won’t be able to save him in this timeline. If you hear the bell ringing at the grave, that means Rubin is still alive and you can save him.

Day 4 — Saving Grief

On the same day, you can also save the pickpocket Grief. To do this, head back to the warehouse district and find warehouse #15, using your map and the building numbers to navigate.

Try to avoid unnecessary confrontations along the way. There are plenty of enemies here, but you can skip direct combat by keeping your distance and scaring off attackers with weapons if needed.

Pathologic 3: How to Save All Characters — Complete Guide

At the warehouse entrance, you’ll be met by two bandits accusing Grief of murder. You have two ways to resolve the situation:

  1. Shoot one of the attackers, and the other will retreat.
  2. Settle things peacefully through dialogue by pointing out the lack of evidence and the senselessness of violence.

To resolve things peacefully, choose the following dialogue options:

  1. What did Grief ever do to you? Why are you after him?
  2. Why do you think Grief killed Simon? Do you have any proof?
  3. That doesn’t answer my question. Is there any evidence Grief is guilty?
  4. That’s it? Seriously?
  5. I didn’t cry when I heard Simon died either. Does that mean I killed him?
  6. Sounds like Simon meant a lot to you. Did you know him personally?
  7. Simon showed by example how to deal with criminals. What about you?
  8. You’re not even sure the killer’s behind that door. You’re just grieving.
  9. That’s not right. But killing Grief won’t bring Simon back, you know that.

Both choices lead to the same outcome. After the conflict, go inside the warehouse and talk to Grief.

Day 7 — The Inquisition’s Trial and Saving All the Accused

The seventh day is one of the most crucial in the game. This is when the Inquisition’s trial takes place, and the fate of several characters is decided.

Pathologic 3: How to Save All Characters — Complete Guide

At the start of the day, you’ll find yourself in an office with the prosecutor, then return to the Abyss. The first time, Dankovsky will be shot, and then you’ll experience a memory from the capital, where the Bachelor meets the Inquisitor Aglaya. Talk to her and choose the last dialogue option. Upon returning to the Abyss, agree to the escort and use the phrase judica causam tuam. This will allow you to proceed to the trial and reach the Cathedral alive.

During the trial, you’ll have to deliver verdicts for each of the accused. It’s best to acquit all six members of the Gorkhon community.

Pathologic 3: How to Save All Characters — Complete Guide

Artemy Burakh can be acquitted if on the third day you made him your assistant at the hospital, on the fifth day stopped the heart extraction, and on the sixth day found him with the children at Lara Ravel’s request.

Pyotr Stamatin survives if you previously found the Polyhedron blueprints. You can get them by returning to day two and digging up the chest at the crow stone in the south of town. Without these blueprints, it’s impossible to acquit him.

The case of Vlad Olgimsky Senior and Junior is closed simply by denying their guilt. Choose dialogue options related to saving the town and making necessary sacrifices.

Alexander Saburov will always be stripped of his rank and will disown Clara as his daughter, no matter what. Don’t pressure the commandant in conversations and pick responses that shut down Karmensky’s accusations.

After the trial, make sure to speak with the Inquisitor again before leaving the cathedral. If you get all four acquittals, you’ll also unlock an achievement.

Day 8 — Saving Vlad Junior

Even after being acquitted at the trial, Vlad Junior can die in a separate story branch. You’ll learn about his death by examining the signs on the walls in the Abyss. After that, head straight to the steppe village in the south of the settlement and talk to the locals.

An elderly woman will tell Dankovsky that the steppe people are holding a bloody ritual in the city. Go there and speak to the girl by the fire. Take a sample of the blood in the cup for analysis and return to the steppe woman. The blood of the young bull will turn out to be Vlad Junior’s. Once you have this information, go to the Womb and talk to Vlad Senior, asking him in detail about his son.

Next, use the clock to return to day six. Head to the Broken Heart tavern and talk to Vlad Junior on the first floor again. It’s important to convince him of his own importance and not to give up his post. The last thing you should say is: It’s a perfectly respectable position. Don’t give it up.

After the conversation, go to the Administration and look for the administrator. Ask him about Vlad and walk behind his desk. On the box above, you’ll find three stacks of letters. Inspect each and read the letters you find. Go into the decree room behind the administrator’s counter through the left door and find the table decorated with bull jewelry. Nearby there will be a locked desk. Interact with it and return to the administrator.

He’ll tell you that the key to the desk is in the room to the right: go to the stairs, turn left, and search the cabinet in the corner. With the key, return to the decree room and open the desk. Examine the handkerchief you find and head to the Kain mansion to see Maria.

Pathologic 3: How to Save All Characters — Complete Guide

Tell the Mistress about Vlad’s situation and ask her to take him under her protection. She’ll agree, and you can return to the Abyss. When you replay the eighth day, you’ll be able to check on Vlad at the Kain mansion and find him alive.

Day 8 — Saving Georgiy Kain

On the same day, you can also prevent the death of Georgiy Kain. After you get the quest related to his project, don’t visit the workshop. Instead, go straight to the Water Tower, where the meeting is taking place.

Pathologic 3: How to Save All Characters — Complete Guide

In your conversation with Georgiy, choose responses that persuade him to postpone the project and listen to those around him:

  1. Georgiy, you should listen to this woman. Otherwise, disaster will strike.
  2. You want to honor Simon’s memory by installing his optical system in the tower according to his last blueprints. You shouldn’t do that.
  3. You taught me to think the way Simon did. You said he knew how to choose both options at once. The best way to honor Simon today is to postpone his project for the sake of the living. They’re asking for simple things, but they’re not stupid or meaningless. They learned something from Simon too.

After the meeting, make sure to talk to him again.

Day 9 — Saving Katerina Saburova

Saving Katerina Saburova is arguably the most difficult and multi-step episode in the game. By the ninth day, Rubin and Artemy Burakh must both be alive, Shabnak must have been banished, all diagnoses must be correct, and you need to have any anti-apathy medication ready—except for morphine.

Pathologic 3: How to Save All Characters — Complete Guide

At the start of the day, Alexander Saburov will ask for your help. Head to their house, examine Katerina, and give the correct diagnosis in the dialogue—ovarian cyst. After that, you’ll need to get consent from Rubin and Burakh to perform the surgery.

Next, go straight to Yulia Lyuricheva, get a blood sample, and check it for compatibility using the analyzer. Use the nearest analysis points at Ipat the water carrier’s place to save time. When you return, start the operation before 2 PM, or Katerina may not survive.

Pathologic 3: How to Save All Characters — Complete Guide

During the operation, administer the medication correctly—restart the day if needed—and then bargain with Death. First, trade resources for a needle. Then trade the needle for a nut, and the nut for an egg. In the end, you need to have seventeen trade points:

  1. Bachelor’s watch — 11
  2. Artemy Burakh as assistant — 1
  3. Stanislav Rubin as assistant — 1
  4. Obtained Yulia Lyuricheva’s blood — 1
  5. Shabnak’s outbreaks destroyed — 1
  6. All diagnoses correct — 1
  7. Sedative administered correctly — 1

After that, talk to everyone present in Saburov’s office and breathe a sigh of relief—Saburova has survived.
